Merge branch 'maint'

* maint:
  git-remote: exit with non-zero status after detecting errors.
  rebase -i: squash should retain the authorship of the _first_ commit
  git-add--interactive: Improve behavior on bogus input
  git-add--interactive: Allow Ctrl-D to exit
